Transforming Cancer Care (TCAT)

Transforming Care After Treatment (TCAT) was delivered in partnership with the People Affected by Cancer, Scottish Government, Regional Cancer Networks, NHS Boards, Local Authorities and the Voluntary Sector from 2013 – 2018. The aim of the programme was to facilitate the development and implementation of future models of care that:

  • Enable people affected by cancer to play a more active role in managing their own care;
  • Provide services which are more tailored to the needs and preferences of people affected by cancer;
  • Give people affected by cancer more support in dealing with the physical, emotional and financial consequences of cancer treatment; and
  • Improve integration between different service providers and provide more care locally.

Success was measured against the following criteria:

    • Initiating and embedding an integrated and sustainable approach to the provision of care involving health, social care and third sector partners that drives a shift in focus from treating the disease to health and wellbeing;
    • Creating a culture of confidence in patients and professionals, which supports people to regain control of their lives, facilitates self-management, develops new approaches to surveillance and reduces unnecessary reviews; and
  • Facilitating the establishment of shared decision-making with patients in cancer follow-up programmes that promotes co-design of high quality, safe and person-centred care.

Regional Cancer Network Involvement

To achieve the overarching aims, the programme was driven through the three regional cancer networks. Support was also provided to secondary care, primary care and local authority partners enabling the review, redesign and testing new approaches within an agreed framework.  Where possible the learning and successful elements of the tests of change were then applied in other areas (different specialties, Boards, or regions).  In total 25 pilot projects were funded as a result of the programme; 11 of which were located within the West of Scotland Cancer Network.

The TCAT Programme was delivered in three distinct phases:

  • Phase 1 projects were funded as a result of the first funding call. These projects tended to be led by secondary care and started in 2014/15;
  • Phase 2 projects were funded as a result of the second funding call and in general are led by primary care or local authorities.       Projects funded during this phase started in 2016;
  • Phase 3 was a transition period which aimed to spread and embed successful work from Phases 1 and 2.
