Transforming Cancer Care (TCC)

Launched in August 2019, the Transforming Cancer Care (TCC) programme builds on the success of both the Transforming Care After Treatment and the Improving the Cancer Journey Programmes. TCC will bring both programmes together with the overall aim being to ensure that people in Scotland diagnosed with cancer are prepared for and supported to live with the consequences of the diagnosis and its treatment. There are many individual elements to the programme including but not limited to, a dedicated link worker for everyone diagnosed with cancer, access to the right person at the right time during and after treatment (risk stratified follow-up), individualised holistic assessment and care planning, early preparation for treatment and ongoing support to deal with the consequences of treatment (prehabilitation and rehabilitation), and the creation and dissemination of a cancer treatment summary. More information about aspects of the programme previously delivered under the individual programmes (TCAT and ICJ) can be found under the relevant subpages. Another page has also been added to share information about the prehabilitation workstream. These pages will develop as the programme progresses.
